APPLICATIONS FOR CHANGE IN CLUB STATUS & CLUB NAME CHANGE
Applications will be accepted from Nov 1 - Nov 30 of each year.

CLUB CHANGE IN STATUS FORM
Club Name Change
- The
request to change a name must be submitted on the Club Letterhead (Old
Club) along with a copy of the minutes of the meeting where they decided
to vote in this new name change.
-complete Club Renewal form with the new club name.
-EMSA will then advise OS of the change
Request for Reinstatement after withdraw or non-renewal of membership
A request for reinstatement must be submitting in writing to admin@emsadistrict.com. Reconsideration for reinstatement will be heard by The EMSA Board at one of its regular meetings, accompanied by a reinstatement fee of $250.
